Abstract
Studies indicate that alterations of physiological signs in children precede in hours the deterioration of the clinical status, therefore the premature identification of risk allows the team an adjustment on the levels for high quality care and assistance. Due to the lack of instruments that provide a quick and correct identification of the child in a critical situation or in a state of clinical deterioration many pediatric score models were evaluated for the alert of its seriousness, and the chosen instrument was the one that better approached our reality, for its simplicity on quick filling out and the possibility of immediate visualization of the results. A systematization of the patients’ vital data record using a simple, but well structured instrument helps the work of the nursing and assistance team in general. It also contributes on offering a safe and high quality attendance, allowing a more adequate distribution of the team according with the necessities of each patient.
